If you check “yes” or “don’t know” to two or more of the risk factors then it’s time to get yourself checked out. How hard can it be?
If you do not know about your family history or your own health then find out! It’s not rocket science.
To Enter The Prize Draw: Simply download
and complete the Men’s Health Scorecard, add your personal details and
take the Scorecard to your health professional to talk about your risks
and have him/her sign it. Even if your score suggests you’re OK, you’re
still eligible to enter the prize draw. All completed Scorecards with
valid personal contact details are eligible for entry into the prize
draw.

Terms and Conditions
of competition:
To be eligible, you must have completed the Men’s Health Scorecard and
provided valid personal contact details.
Completed Scorecards must be received by the Cancer Society by 30 November
2008. Prizes will be drawn on 3 December 2008. Winners will be advised
by telephone. No correspondence concerning this prize draw will be entered
into. Prizes cannot be exchanged for cash equivalents. By accepting the
prize, winners accept that the Cancer Society of New Zealand may use
pictures and stories about the winners. Any information collected by
the Cancer Soceity will be retained for ther use only.
